All service providers are trained and autism certified and are vulnerable sector screened, insured and maintain strict safety protocols and techniques designed for children. We take great care in the use of any tools and products while providing our services. However, there may be the possibility of a reaction or distraction that could result in small risk of accidental nick or cut. Services may be declined or modified based on various health conditions or contraindications. Service providers have the right to refuse or stop service at any time if deemed unsafe for the provider or client.

CLIENT WAIVER
I am the legal caregiver or guardian of the above-named children under the age of 18. I understand I must always accompany the child during the service. I have read, understand and approve of the information provided above and have been advised of risks associated with the use of the services and products. I agree to the terms of service and waiver knowing any risks associated and give my full consent to have the services, procedures and products use on myself or children under my care. By agreeing to these terms of service, and waiver I irrevocably release the company, its officials, directors, shareholders, providers, employees, representatives or consultants from any claim, action, cause of action, damage or liability associated with the services and products being provided or recommended by Lice Squad Canada Inc.
